Intrauterine insemination or IUI is a process in which a male partner’s sperms are taken and washed in the laboratory. After that, they are placed in the uterus of the female partner. It makes it easy for the sperm to reach the uterus and achieve pregnancy. IUI can be used in the following situations.

Donation in infertility refers to third-party reproduction,
Sperm donation: In artificial insemination, the donated sperm is placed in a woman’s uterus or adding the sperm to a woman’s eggs outside of the body.
Egg donation involves using donor eggs, either provided by a family member or friend for free or purchased, in an IVF cycle. These donated eggs are fused with the male partner sperm or donated sperm. Then the embryo formed is placed into the uterus of the patient.
Embryo donation Some individuals who have undergone fertility treatment will donate their unused embryos for another person or couple’s use. The embryo can then be implanted into the recipient mother so she can become pregnant and give birth.
Laparoscopy/ Hysteroscopy are surgical procedures used to diagnose and treat. Endometriosis, uterine fibroids, endometrial polyps, uterine anomalies or pelvic infection. They can also correct congenital abnormalities such as uterine septum or adhesions that can cause miscarriage or premature labour.Using hysteroscopy improves fertility and increases pregnancy rates. Laparoscopy has shown to improve IVF outcomes by 50 per cent.
In some cases, correction of these abnormalities alone will result in spontaneous pregnancy. Hysteroscopy and laparoscopy can also be performed before an in vitro fertilization cycle to improve IVF success.
Diagnosing male infertility problems involves:
General physical examination and medical history: This includes examining your genitals and asking questions on any inherited conditions, chronic health problems, illnesses, injuries or surgeries that would affect fertility.
Semen analysis. Semen samples are obtained by masturbating and ejaculating them into a container at the doctor's office.
Your semen is then sent to a laboratory to live the amount of sperm present and appearance for any abnormal shape (morphology) and movement (motility) of the sperm.
As sperm counts fluctuate, several semen analysis tests are done over some time to ensure an accurate result.
Some additional tests to help identify the cause of your infertility:
In women, much of the endometrium is shed and regenerated monthly during the cycle . The endometrium is divided into two layers, epithelial cells and stromal cells. It undergoes remarkable histological and structural changes throughout the menstrual cycle in preparation for embryonic implantation and subsequent shedding and regeneration. Thin endometrium adversely affects pregnancy success rates with fertility treatment. Several treatment modalities are presented to patients with thin endometrium to enhance endometrial thickness.
Cryopreservation is a technique used to freeze and then thaw eggs, embryos or sperm for future use in IVF cycles. Thawed sperm are in use in IUI (intrauterine insemination) treatment cycles. A woman doesn't need to undergo stimulation to have an embryo transfer during an infertility treatment cycle.
Ovarian stimulation: The fertility specialist monitors the timing of egg release, the hormonal level and other conditions for IVF. The doctor keeps a watch over produced eggs are appropriate or not.
Egg retrieval: In this step, eggs are retrieved from the female’s ovary using a needle under light sedation. Further, placing the eggs in a dish containing nutrient media, then shifted to the incubator.
Fertilization: The most active sperm fertilizes with the egg in a special chamber. The process is under observation so that a healthy embryo can develop.
Embryo transfer: The healthiest embryo gets implanted under observation. With the help of small plastic tubes, embryo transfer takes place.
IVF involves three parts:
(1) One day for checkup / pre-IVF preparation. Based on what reports are available with the couple, the doctor may prescribe additional tests and investigations to ensure that the couple really need IVF and that they are ready to start that.
(2) 15 days for IVF procedure until eggs are picked up
(3) 15 days for the embryo transfer procedure, where the frozen embryo is thawed and transferred to the uterus
No, IVF is not painful because the injections for IVF are purified and injected in subcutaneous form. The egg collection process is done under light sedation and is painless. At the time of embryo transfer, it is a 2 to 5 minutes process that hardly causes any pain.
In women, fertility is at its peak in their 20s. It starts declining after 30 years of age. After 35, the decline is steep. Women aged 35 and above have a lower number of eggs available for fertilization, the quality of the eggs are also much lower.
Low sperm count can also be known as oligospermia. One may undergo ICSI (Intracytoplasmic Sperm Injection), single sperm gets injected into the matured egg. This treatment increased the chances of conceiving. Besides this, low sperm count can also be treated by medicines and hormone management in a few patients.
